Maternal autonomy based obligations of physician.
A mother is entitled to receive care during pregnancy, known as Antenatal care (ANC) is a health service to pregnant women during pregnancy (department of health, 1994).During pregnancy consists of three times checks up to the clinic or hospital.
Health workers provide midwifery care during childbirth involving a client or family, include:
Assessing the needs of the client's midwifery care during childbirth.
b. Determine diagnosis and need for midwifery care during childbirth.
c. Midwifery care plan with the client according to the priority issues heard.
d. Implement midwifery care in accordance with the plans that have been prepared.
e. Evaluate the care that has been supplied with the client.
f. Make a plan of action on the mother during labor in accordance with priority.
g. Make midwifery care.
But patient’s rights are up held and her autonomy is respected when she is given the freedom to choose among the available treatment options based on her values and beliefs.

Medical practitioner or midwife is able to provide good service for mothers with many of the midwife's own strategy. Such as building trust in the mother, communicate well with understanding the culture of mother. So that the mother will feel comfort in the treatment or care of a midwife.

Fetal beneficence based obligations of mother
The mother and fetus are involved in a symbiotic relationship in which the mother is the moral guardian. If significant differences arise in the interests of the mother and fetus, the mother has the responsibility to consider the interests of both in making an informed decision regarding medical treatment. If a conflict arises, the competent mother’s rights to personal autonomy should prevail over the lesser rights of the fetus early in gestation.
Fetal beneficence based obligations of physician
Medical personnel, especially midwives play a role in the care of the fetus, especially in terms of:
A. Conduct the assessment / examination fetal growth and development, including:
